Request a Sick Note
Up to 1 Week
If you are off work due to ill health for up to a week, you should
‘self-certify’ using self certificate form SC2 which can be obtained
from your employer. Alternatively, you can obtain these forms from the
local Benefits Agency office.
Legally, you do not require a medical certificate from your doctor for
absences of up to 1 week but some employers now insist on a sick note
from the doctor even for short absences. If you need a medical
certificate in these circumstances, the doctor will charge a fee for
providing a private certificate and this fee must be paid either by you
or your employer. Our Reception team will be able to tell you how much
private medical certificates cost.

After 1 Week
If your illness keeps you off work for more than 1 week, you will
require a medical certificate from the doctor and you should therefore
contact our Reception team in the first instance. There is no charge for
these certificates.
