Access to Medical Notes
Under the Data Protection Act
1998 everyone has a right to have a copy of their medical records. There
are some exceptions:
- Access may be refused if the
information contained in the records is deemed detrimental to the
health of the patient or another person
- Details about third parties may be
removed
- If applying for another person you
must have their written consent or Power of Attorney
Medical records have been
computerised at our surgery. Old paper notes are in the process of
being transferred to the computer system.
Notes from treatment received elsewhere, such as hospitals, are
retained there but we usually receive a summary report of your
treatment which is added to your records.
If you wish to access your records please apply in writing to the
Practice Manager. You will receive a response within 21 days.
A charge of £10 is made if you require information that is held on
the computer. If you also wish to access manual records the charge
can be up to £50 for the provision of paper copies. Individual
sheets are charged at 49p page.
