New Patients
We are pleased to welcome new patients to the
Practice list. In order to register, please enquire at Reception where
you will be given a purple and white registration form and a New Patient
Questionnaire to complete. Alternatively, these forms can be downloaded
by clicking on the links below:

New
Patient Questionnaire
When completed, these forms should be returned to our
Reception team.
We will also need to see proof of your identity.
You will be asked to provide the following:
- ONE type of photo identity, such as your passport or driving
licence
- TWO types of proof of your address, such as a recent utility
bill (gas, electricity, water or phone bill, but not a mobile phone
bill) or council tax bill, rental contract or recent bank statement.
You will then be asked to make an appointment to see a member of our
Practice Nursing team for a new patient check.
Temporary Residents
You are entitled under NHS regulations to see a doctor anywhere in the
United Kingdom if you are away from home and need medical help. We are
happy to see anyone who is staying in our catchment area on a temporary
basis. You will be asked to complete a Temporary Resident form before
seeing the doctor or nurse.
